Recognition for Practice Improvement

We were pleased to welcome the Hon Dr David Clark, Minister of Health, to Johnsonville Medical Centre last week, to recognise our achievement of receiving a Certificate of Achievement for our role in the Health Care Home programme.

Health Care Home is a development programme that is supporting us to make changes to the way we support our patients, and as a result we're seeing significant improvements in the way patients can interact with us. Our community of patients are being seen less in ED than before, and are able to access the support they need in more cost effective and timely ways than ever before. 

Alongside this work we have seen an 80% decrease in number of calls we were unable to answer, and have also developed relationships and services with the DHB which enables our patients to receive care at the practice which would previously require admission to hospital.

A significant factor in the success of Health Care Home is ManageMyHealth, our online service. ManageMyHealth allows you to book appointments, request repeat prescriptions, see your clinical notes, and email your GP, making it quicker and easier to access as required. 

The Health Care Home programme has been well supported by the Capital and Coast District Health Board, and we appreciative of their support in helping us achieve better health outcomes for our patients.
